Now that you have submitted your registration information, you have three more steps.

1) Signature Form

Print this form and sign and date it as instructed. The final item on this form requires a chorister and parent/guardian signature after reading the Chorister Agreement (Click here to read it.)

2) Payment Form (choose your choir)

The following forms are for joining the choir mid-season.

• Prelude and Intermezzo
• Fiero and Lyrica
• Aria and Exit 194

Print out the form for your choir, choose your method of payment, and provide all requested information. (If you are enrolling as a dual member of Lyrica/ARIA or Fiero / Exit 194, use the Fiero and Lyrica form.)

3) Turn in these forms and payment.

The best time to finish this process is at the Registration Bar-B-Q on September 11. If you are choosing the Installment Plan using personal checks, you must submit the post-dated checks with your payment form in order to complete your registration.
